Job Summary
The purpose of this role is to work as a hospital pharmacist on all parts of the pharmacy service Patient care, Operations and Quality Assurance service. They provide the main pharmacist input on a day to day basis and may, in the absence of a senior pharmacist, deputise for the Pharmacy Supervisor of their section as required. They provide clinical pharmacy services as a part their working day. They manage their manpower resources in accordance with HMC policies on Human Resources. The job-holder will consistently work to an advanced level, exceeding the performance expected of an entry to practice Pharmacists in all their work.

Specific Responsibilities:
Patient care
Verify physician orders for appropriateness according to HMC policies and procedures.
Gather required patient information.
Maintains and updates a patient profile with demographics, diagnosis, allergies, and current medications.
Identify, prioritize and resolve drug related problems.
Uses appropriate communication skills, and effectively communicates with patients, physicians, and nurses to solve identified drug related problems.
Provide patient education (e.g., counselling information and education on adherence issues, either verbal or written).
Monitor medication effectiveness by review of the medical record and direct questioning of the patient or their carer.
Monitors drug therapy to evaluate appropriateness of use, drug, dosage form, regimen, route, therapeutic duplication, and drug interaction.
Professional Collaboration and Team Work
Participate in the pharmacy shift and on-call system for their facility
Cooperate with and show respect for all members of the inter-professional team.
Refer patients to other health care providers when required.
Work with other health care professionals to promote health and wellness in the community.
Participating in collaborative health related research.
Detects, monitors, documents, and reports adverse drug reactions and medication errors and near miss.

Drug, Therapeutic and Practice Information
Provides drug information to health care professionals and to patients and their carer
Utilize medical references and evaluate scientific information to answer drug information questions
Identifies and acts upon cost avoidance in drug therapy, by promoting cost effective drugs and by reducing waste.

Demonstrate sensitivity, respect and empathy when communicating with diverse groups or individuals.
Participate in health promotion activities.
Drug Distribution
Interpretation of drug orders.
Performance of pharmaceutical calculations.
Insures proper technique and accurate preparation of all pharmaceutical products.
Ensure proper storage and handling conditions.
Acquiring and disposing of drugs.
Checks accuracy of prescriptions filled by technicians.
Checks all pharmaceutical products prepared by technicians.
Insures timely and accurate dispensing of prescriptions.
Insures compliance with narcotic and controlled substance related polices and procedures
Assign technician duties.
Understanding Management Principles
In the absence of a senior pharmacist, deputize for their Pharmacy Supervisor e.g. in the meetings of the pharmacy senior management team.
Guide and supervise the Junior Pharmacists, and students who work within their service.
Ensure the consistency of service to patients and other stakeholders by assisting in the evaluation of the [P/T/QA] service.
For technicians and Junior Pharmacists, provides input to the initial assessment, departmental appraisal, personal development need.
Monitor and reports medication errors and near miss, adverse events and complaints that are identified by or involve [P/T/QA] service and works the supervisor to resolve the issue and minimise future risk
Attends and assists with the end of the fiscal year inventory process.
Assists with the maintenance and care of department equipment and supplies
Perform other duties as assigned

Education
Bachelor Degree in Pharmacy from recognized program
