About the Role

We're seeking a friendly & professional Social Worker to work on a unique 3 month project (with view potential view to go permanent) which focuses on innovating changes to how customers interact with practices and creating in-practice behaviours to improve the provider and patient experience and ultimately keep people out of the hospital. We are running the pilot in our practices across Parramatta, Auburn and Campbelltown.

  • Review patient files and develop advice to GPs that could assist patients with access to Community Service
  • Follow-up actions from the GPs following a Case Review.
  • Engage with patients
  • Work in collaboration with a Registered Nurse to manage the prep, support and follow-ups of any case reviews.

What we are looking for

  • Current understanding of social services (both in Western Sydney and Nationally such as ACAT)
  • Ability to work independently and as part of a team
  • Understanding (and preferred experience) with health coaching
  • Work independently and as a collaborative member of the multidisciplinary team
  • Able to prioritise competing demands
