The PHO is on a mission with our partners; Te Whatu Ora Te Tai o Poutini, and Poutini Waiora, to transform the experience of tāngata whaiora on the West Coast. We want consumers and their whānau to experience improved access to care and we will do this by improving collaboration between providers. This Primary Mental Health Clinical Liaison / Lead will:
- Develop a whole-of-system model of care across the continuum of mental health services. This will encompass a single point of entry approach and a shared triage service.
- Work with our partners to make visible pathways to kaupapa Māori and Rongoa Māori services.
- Create improved access to advice from specialist mental health staff for primary and community clinicians.
- Examine opportunities to shift some specialist care services into primary or community care settings.
